Gözlük ya da lens reçetenizde, sferik değerin yanında CYL (silindirik) ve Eksen (Axis) diye değerler görmüşsünüzdür. Bu değerler astigmatizmayı anlatır. Astigmatizma çok yaygındır, genellikle miyopi ya da hipermetropiyle birlikte bulunur. Dışarıdan gelen ışık önce gözün saydam ön tabakası korneadan, ardından göz içindeki mercekten geçer ve gözün arka duvarındaki retina üzerinde tek bir noktada buluşur. Görüntü tam retinaya, tek noktaya düştüğünde net görürüz.
Bunun için korneanın yüzeyi her yönde eşit kavisli olmalıdır — tıpkı yuvarlak bir topun (kürenin) yüzeyi gibi. Astigmatizma tam da bu noktada devreye girer.
Astigmatizma nedir?
Astigmatizmada kornea (bazen göz içindeki mercek) her yönde eşit kavisli değildir. Bir yönde daha dik, ona dik olan yöndeyse daha düzdür. Yani kornea, yuvarlak bir top yerine ragbi ya da Amerikan futbolu topu gibi ovaldir.
Bu eşitsizlik yüzünden ışık tek bir noktada toplanamaz; farklı yönlerden gelen ışınlar farklı yerlerde odaklanır ve görüntü retinaya net düşmez. Sonuç: her mesafede (hem uzakta hem yakında) hafif bulanık, gölgeli ya da hafifçe esnemiş görme.
Astigmatizma nasıl oluşur?
Astigmatizmanın büyük çoğunluğu doğuştan gelir ve korneanın doğal şekliyle ilgilidir. Kesin nedeni tam olarak bilinmez ve önlenebilir bir kusur değildir. Çocuklukta ya da gençlikte ortaya çıkabilir, yıllar içinde az miktarda değişebilir.
Daha seyrek olarak; göz yaralanması, göz ameliyatı ya da korneanın incelip giderek düzensizleştiği keratokonus gibi durumlar da astigmatizmaya yol açabilir.
Astigmatizma genel olarak ikiye ayrılır:
Düzenli astigmatizma: — En sık görülen tiptir. Korneanın en dik ve en düz yönleri birbirine diktir; gözlük ya da uygun lensle düzeltilir.
Düzensiz astigmatizma: — Kornea yüzeyi düzensizdir (örneğin keratokonus ya da yara izi sonrası).

Nasıl fark edilir?
Astigmatizmanın belirtileri kişiye ve dereceye göre değişir:
Hem uzakta hem yakında bulanık, gölgeli ya da çift hatlı görme.
Göz yorgunluğu, baş ağrısı, gözleri kısarak bakma.
Gece araç kullanırken ışıkların etrafında halelenme ve parlama.
Hafif astigmatizmada hiçbir belirti olmayabilir.
Çocuklarda astigmatizmayı fark etmek zor olabilir; bu nedenle düzenli göz taraması önemlidir.
Astigmatizma reçetede bir güç ve bir açı ile gösterilir:
Silindirik (CYL): — Astigmatizmanın derecesini (gücünü) gösterir; sferik değer gibi diyoptri (D) biriminde yazılır.
Eksen (Axis): — Kusurun hangi yönde olduğunu gösterir; 0–180° arasında bir açıdır. (90° dikey, 180° yatay yönü anlatır.)
Kısaca: sferik değer gözün genel odağını, silindirik değer yöne bağlı farkı, eksen ise bu farkın hangi yönde olduğunu söyler. Birçok gözde sferik ve silindirik değer birlikte bulunur.
Silindirik (torik) gözlük camı: — En yaygın çözümdür; ışığı yalnızca gereken yönde fazladan kırar.
Torik kontakt lens: — Astigmatizmaya özel tasarlanmış lenslerdir.
Düzensiz astigmatizmada: — Sert gaz geçirgen ya da özel lensler gerekebilir.
Uygun durumlarda: — Lazer ya da cerrahi yöntemlerle korneanın şekli düzeltilebilir.
Hangi yöntemin uygun olduğuna, astigmatizmanın derecesine ve gözün yapısına bakarak göz hekimi karar verir.
Astigmatizma neden takip edilir?

On your glasses or contact lens prescription, next to the spherical value, you have probably seen the columns CYL (cylinder) and Axis. These describe astigmatism. Astigmatism is very common and usually occurs together with myopia or hyperopia. In this short article we explain, in plain language, what astigmatism is, how it arises in the eye, how it is noticed, and how it is corrected.
A quick reminder: how does the eye see clearly?
Incoming light first passes through the eye's clear front layer, the cornea, then through the lens inside the eye, and comes together at a single point on the retina, the layer at the back of the eye. When the image falls on the retina as a single point, we see clearly.
For this, the surface of the cornea must be equally curved in every direction — just like the surface of a round ball (a sphere). Astigmatism is exactly where this breaks down.
What is astigmatism?
In astigmatism the cornea (and sometimes the lens inside the eye) is not equally curved in every direction. It is steeper in one direction and flatter in the perpendicular direction. So instead of a round ball, the cornea is oval — shaped more like a rugby or American football.
Because of this inequality, light cannot be gathered to a single point; rays coming from different directions focus at different places, and the image does not fall sharply on the retina. The result is slightly blurred, shadowed or stretched vision at all distances (both far and near).
How does astigmatism arise?
Most astigmatism is present from birth and is related to the natural shape of the cornea. Its exact cause is not fully known and it is not a preventable defect. It can appear in childhood or young adulthood and may change slightly over the years.
Less often, an eye injury, eye surgery, or conditions such as keratoconus — in which the cornea thins and becomes progressively irregular — can also lead to astigmatism.
Astigmatism is generally divided into two types:
Regular astigmatism: — The most common type. The steepest and flattest directions of the cornea are perpendicular to each other; it is corrected with glasses or suitable lenses.
Irregular astigmatism: — The corneal surface is irregular (for example after keratoconus or scarring).

How is it noticed?
The symptoms of astigmatism vary from person to person and with its degree:
Blurred, shadowed or doubled vision at both far and near distances.
Eye strain, headache, squinting to see.
Halos and glare around lights when driving at night.
With mild astigmatism there may be no symptoms at all.
Astigmatism can be hard to notice in children; for this reason regular eye screening is important.
Astigmatism is shown on a prescription with a power and an angle:
Cylinder (CYL): — shows the degree (power) of the astigmatism; like the spherical value, it is written in diopters (D).
Axis: — shows which direction the defect lies in; an angle between 0 and 180°. (90° means the vertical, 180° the horizontal direction.)
In short: the sphere describes the eye's overall focus, the cylinder the direction-dependent difference, and the axis the direction in which that difference lies. In many eyes the spherical and cylindrical values occur together.
How is it corrected?
Astigmatism is corrected with lenses that add extra focusing power only in the direction that is under-refracted:
Cylindrical (toric) spectacle lens: — the most common solution; it refracts light extra only in the needed direction.
Toric contact lens: — lenses designed specifically for astigmatism.
In irregular astigmatism: — rigid gas-permeable or special lenses may be needed.
In suitable cases: — laser or surgical methods can reshape the cornea.
Which method is appropriate is decided by the ophthalmologist, based on the degree of the astigmatism and the structure of the eye.
Why is astigmatism tracked?
In most people astigmatism is relatively stable; however, it can change over time, especially in children and young people or in conditions such as keratoconus. Following the cylindrical value and the axis over the years is important for catching a possible change early.
