Katarakt ameliyatı bugün dünyada en sık yapılan ve en başarılı cerrahi girişimlerden biridir. Genellikle 10-15 dakika sürer, ağrısız geçer ve aynı gün eve dönülür.

Ama ameliyathaneden çıkmak işin yarısı bile değil. Gözünüzün içine yeni bir mercek yerleştirildi ve korneada (gözün şeffaf tabakası) milimetrik bir kesi var; bunların iyileşmesi birkaç hafta sürüyor. Bu sürede yapacaklarınız sonucu doğrudan etkiliyor.

Ameliyattan hemen sonra: ilk 24 saat

Ameliyattan sonra ilk 1-2 saat sırtüstü dinlenin. Sonrasında ayağa kalkabilir, yürüyebilir, televizyon izleyebilir ve istediğinizi yiyebilirsiniz.

İlk gün yapın

Görmeniz neden hemen netleşmiyor?

Ameliyattan çıkınca "hemen net göreceğim" beklentisi çok yaygındır. Oysa görme kademeli düzelir ve bunun anlaşılabilir bir sebebi vardır.

Kataraktlı mercek, gözün içinde ses dalgalarıyla eritilerek çıkarılır. Gözün ön yüzündeki saydam tabakanın (kornea) iç yüzeyinde, bu tabakayı berrak tutmakla görevli bir hücre katmanı vardır. Ameliyat sırasında bu hücrelerin bir kısmı etkilenir ve kornea geçici olarak ödemlenir — yani hafifçe su tutar. Su tutan kornea, buğulanmış bir cam gibi davranır.

Katarakt ne kadar sertse eritmek için o kadar çok enerji gerekir, ödem de o kadar belirgin olur. Bu yüzden çok olgunlaşmasını beklemeden ameliyat olmak iyileşmeyi kolaylaştırır.

Ödem birkaç gün içinde kendiliğinden çözülür ve görme netleşir. Damlalarınız bu süreci hızlandırır.

Damlalarınız

Katarakt ameliyatından sonra genellikle üç tip damla verilir:

  • Antibiyotik damla — enfeksiyonu önler.
  • İltihap önleyici damla (kortizonlu ve/veya kortizonsuz) — ameliyatın gözde bıraktığı iltihabı söndürür. Genellikle dozu azaltılarak kesilir.
  • Suni gözyaşı — batma ve kuruluk için.

Damlatma kuralları

  • Ellerinizi yıkayın — , her seferinde.
  • Alt göz kapağınızı hiç bastırmadan hafifçe aşağı çekip küçük bir cep oluşturun — , damlayı oraya damlatın.
  • Şişenin ucunu hiçbir yere değdirmeyin — göze, kirpiğe, parmağınıza.
  • Tek damla yeterlidir. — Gözünüz fiziken tek damladan fazlasını tutamaz. Dışarı akan kısmı yanağınızdan silin.
  • İki farklı damla arasında en az 5 dakika bekleyin. — Peş peşe damlatırsanız ikincisi birincisini dışarı yıkar.

"Gözüm iyileşti" diye damlaları erken bırakmak en sık yapılan hatadır. İltihap gözle görülür bir şikâyet vermeden de sürebilir. Damla programını hekiminiz bitirir, siz değil.

Kontrolleri kaçırmayın

Kortizonlu damlalar bazı kişilerde göz tansiyonunu yükseltir. Bu, hiçbir belirti vermeden olur ve yalnız muayenede fark edilir. Kontroller genellikle ilk gün, birinci hafta ve birinci ay olmak üzere planlanır; hekiminiz gerekirse sıklaştırır.

Batma hissi geçmiyorsa

Ameliyattan sonra birkaç gün batma ve kum hissi olması beklenir. Ama haftalar sonra da sürüyorsa sebebi genellikle ameliyatın kendisi değildir:

Güneş gözlüğü: İlk günlerden itibaren dışarıda UV korumalı, geniş çerçeveli bir güneş gözlüğü kullanın. Yeni merceğiniz ışığı eskisinden çok daha fazla geçirir; bu hem konfor hem koruma meselesidir.

Bu belirtilerde beklemeyin, arayın

Ameliyat sonrası hafif rahatsızlık normaldir. Aşağıdakiler normal değildir ve aynı gün değerlendirilmelidir:

  • Görmede ani kayıp veya azalma
  • Ağrı kesiciye rağmen geçmeyen şiddetli ağrı
  • Işık çakmaları
  • Sayılamayacak kadar çok sayıda uçuşan cisim — (sinek uçuşması)
  • Görmenin bir köşesinde perde veya gölge

Bu belirtilerin arkasında göz içi iltihabı, retina dekolmanı, göz tansiyonu yükselmesi veya sarı nokta ödemi olabilir. Hepsi çok nadirdir — ama hepsinde de erken müdahale sonucu belirler.

"Acaba doktoru boşuna mı rahatsız ediyorum" diye beklemeyin. Bu belirtilerde saatler önemlidir. Hekiminize ulaşamıyorsanız en yakın göz acil servisine gidin.

Gözlüğünüz ve ikinci gözünüz

Gözlük için 4-6 hafta bekleyin. Ameliyat kesisi tam iyileşmeden ölçülen numara doğru çıkmaz. Bu arada eski gözlüğünüzü koruma amaçlı takabilirsiniz.

Neden hâlâ küçük bir numaram var?

Takılacak merceğin gücü ameliyat öncesi ölçümlerle hesaplanır. Ölçüm cihazları çok hassastır, ama sonuç matematiksel bir tahmindir — her gözün iyileşme yanıtı biraz farklıdır. Çoğu hastada sonuç +0.50 ile −0.50 arasında çıkar ve bu değerler için gözlük gerekmez. Küçük bir sapma bir hata değil, işin doğasıdır.

Astigmatınız varsa

Tek gözünüz ameliyat olduysa iki göz arasındaki numara farkı sizi rahatsız edebilir. Geçici çözüm: eski gözlüğünüzün ameliyatlı taraftaki camını çıkartın. İkinci gözde katarakt varsa genellikle 1-4 hafta sonra ameliyat edilir.

Aylar sonra görme yeniden bulanırsa

Katarakt ameliyatından aylar veya yıllar sonra görmenin yeniden buğulanması sık karşılaşılan bir durumdur. Hastalar bunu genellikle "kataraktım tekrarladı" diye anlatır.

Katarakt tekrarlamaz. Çıkarılan mercek geri gelmez. Olan şey, takılan merceği yerinde tutan ince ve şeffaf zarın zamanla matlaşmasıdır.

  • Görme yavaş yavaş buğulanır, ışıkta parlama artar — kataraktın kendisine benzer.
  • Çözümü YAG lazer işlemidir: — birkaç dakika sürer, ağrısızdır, damla ile yapılır, ameliyathane gerektirmez.
  • Bir kez yapılır; aynı gözde tekrarlamaz.

Cataract surgery is one of the most common and successful surgical procedures performed today. It usually takes 10 to 15 minutes, is painless, and you go home the same day.

But leaving the operating room isn't even half the job. A new lens has been placed inside your eye, and there's a tiny incision in your cornea (the clear front surface of the eye); healing takes a few weeks. What you do during this time directly affects the outcome.

The good news: the rules are short, and most of them only apply for the first two weeks.

This article provides general information. Your surgeon's instructions always take priority; if there's a conflict, follow what your doctor advises.

Right after surgery: the first 24 hours

Rest lying on your back for the first 1-2 hours after surgery. After that, you can get up, walk around, watch television, and eat whatever you like.

These are normal, don't worry

  • A gritty or scratchy feeling in the eye
  • Mild redness and watering
  • Sensitivity to light
  • Blurry vision — due to the medications used during surgery and the bright surgical light
  • Seeing halos around lights
  • Colors looking brighter, even slightly blue-tinted — you'd been looking through a lens that had yellowed over the years, and now you're looking through a clear one. This usually starts to feel normal within a few days.

Do on the first day

  • Rest. You don't need to keep your eye closed.
  • Watching television, using your phone, and reading are fine; take breaks when you get tired.
  • Wear the protective pad or shield as your doctor instructs. Its purpose is to keep you from rubbing your eye without realizing it.
  • You can take paracetamol if you have pain. — If you're taking blood thinners, don't stop or start them on your own — ask your doctor.

Don't do on the first day

  • Don't rub your eye. — This is the single most important rule on this list.
  • If you need to wipe your eye, wait for the tear to run down to your cheek and wipe it from there. — Don't touch the eyelid itself.
  • Don't drive.
  • Don't let water get into your eye.
  • Don't bend forward with your head below waist level.
  • Don't lift heavy objects, strain, or squeeze your eye shut.
  • Don't sleep on the side of your operated eye.
  • Avoid forceful coughing.

Why doesn't your vision clear up right away?

It's very common to expect to see clearly the moment surgery is over. In reality, vision improves gradually, and there's a clear reason why.

The clouded lens is broken up using sound waves and removed from the eye. On the inner surface of the clear layer at the front of the eye (the cornea), there's a layer of cells responsible for keeping that layer clear. Some of these cells are affected during surgery, and the cornea temporarily swells — meaning it retains a bit of fluid. A cornea holding fluid behaves like a fogged-up piece of glass.

The harder the cataract, the more energy is needed to break it up, and the more noticeable the swelling. This is why having surgery before the cataract becomes too advanced makes recovery easier.

The swelling resolves on its own within a few days and vision clears up. Your eye drops speed up this process.

Day-by-day recovery

The first few hoursVision is noticeably blurry — due to the drops and the surgical light. Time to rest.
Day 1-2Blurriness may continue because of corneal swelling. Your first check-up happens now, and your drop schedule is set.
From day 3 onwardVision noticeably starts to clear.
Week 1Most of your vision has settled. You may still feel some blurriness during this period — usually because your glasses prescription hasn't been finalized yet. If you see well with trial lenses at your check-up, everything is fine.
Week 2Daily life returns to normal.
By the end of month 1Vision should have reached its final level. Your glasses prescription is measured. If it's still not where it should be at this point, your doctor will look into why.

This timeline is an average. It can vary depending on how advanced your cataract was and any other conditions in your eye (macular degeneration, glaucoma, diabetes).

Your eye drops

After cataract surgery, you're usually given three types of drops:

  • Antibiotic drops — prevent infection.
  • Anti-inflammatory drops (steroid and/or non-steroid) — calm the inflammation surgery leaves in the eye. The dose is usually tapered down before stopping.
  • Artificial tears — for grittiness and dryness.

How to use your drops

  • Wash your hands — , every time.
  • Gently pull your lower eyelid down without pressing on it to form a small pocket, — and drop the medication there.
  • Never let the bottle tip touch anything — your eye, eyelashes, or fingers.
  • One drop is enough. — Your eye physically can't hold more than one drop; wipe away any overflow from your cheek.
  • Wait at least 5 minutes between two different drops. — Using them back-to-back washes the first one out with the second.
  • Keep your eye closed for 1-2 minutes after each drop, — and gently press the inner corner near your nose with your finger.
  • If you're using a gel or ointment, apply it last.

You don't need to wake up during the night to use your drops.

"My eye feels better" is the most common reason people stop their drops too early. Inflammation can continue without any visible symptoms. Your doctor decides when the drop schedule ends — not you.

Don't skip your check-ups

Steroid drops raise eye pressure in some people. This happens without any symptoms and can only be detected during an exam. Check-ups are usually scheduled on day 1, at week 1, and at month 1; your doctor may schedule them more often if needed.

If the gritty feeling doesn't go away

It's normal to feel some grittiness for a few days after surgery. But if it continues for weeks, the cause usually isn't the surgery itself:

  • The drops themselves — can irritate or cause an allergic reaction in some people.
  • Pre-existing dry eye — often becomes more noticeable after surgery.
  • A dry climate — makes things worse.

Have your doctor check first. If there's no surgery-related problem, the solution is artificial tears; there are further treatment options if the grittiness doesn't improve despite the drops.

When can you get back to your usual activities?

  • Water contact with the eye: — No water in the eye for the first week.
  • Bathing: — Everything below the neck is fine. Wash your hair by tilting your head back so dirty water doesn't run into your eye. Normal bathing is usually possible after week 1 — follow your doctor's advice.
  • Washing your face: — Wipe with a damp cloth instead of splashing water.
  • Screens, reading, television: — Fine from day one.
  • Returning to work (desk job): — 2-3 days.
  • Returning to work (dusty, physical, outdoor): — 10 days, or with your doctor's approval.
  • Driving: — Don't, until your doctor approves.
  • Running, cycling, the gym: — About 1 week.
  • Heavy lifting or straining activities: — 2 weeks.
  • Swimming, the sea, pools, saunas: — 4 weeks.
  • Eye makeup: — 4 weeks.
  • Dust, smoke, gardening, farm work: — 2-3 weeks.
  • Sexual activity: — About 1 week.
  • Air travel: — No restriction — cataract surgery doesn't prevent flying.

Sunglasses: Wear UV-protective, wide sunglasses outdoors from the first days on. Your new lens lets in far more light than your old one did — this is both a comfort and a protection matter.

Bending forward: if you pray in a position that involves bowing to the ground, doing so seated instead is recommended for the first week. When picking something up from the floor, squat instead of bending over.

Don't wait on these symptoms — call your doctor

Mild discomfort after surgery is normal. The following are not, and should be evaluated the same day:

  • Sudden loss or decrease in vision
  • Severe pain that doesn't respond to painkillers
  • Flashes of light
  • An overwhelming number of floaters — (specks drifting across your vision)
  • A curtain or shadow in one part of your vision
  • Increasing redness — , yellow-green discharge from the eye
  • Severe eye or head pain along with nausea and vomiting
  • A blow or impact to your eye

These symptoms can point to inflammation inside the eye, retinal detachment, elevated eye pressure, or swelling at the macula. All of these are very rare — but in every case, early treatment is what determines the outcome.

Don't hold back out of worry that you're bothering your doctor unnecessarily. Hours matter with these symptoms. If you can't reach your doctor, go to the nearest eye emergency department.

Your glasses and your other eye

Wait 4-6 weeks for glasses. If your prescription is measured before the incision has fully healed, it won't be accurate. In the meantime, you can wear your old glasses just for a sense of comfort.

Why is my prescription still slightly off?

The power of the lens that gets implanted is calculated from measurements taken before surgery. The measuring devices are very precise, but the result is still a mathematical prediction — every eye responds to healing a little differently. Most patients end up somewhere between +0.50 and -0.50, and glasses aren't needed for values in that range. A small deviation isn't a mistake; it's simply how this works.

If you have astigmatism

A standard lens doesn't correct astigmatism; even if your surgery goes very well, you'll still need glasses. Toric lenses, which correct astigmatism, exist for this reason and are planned before surgery if suitable for your eye.

Reading glasses

With a standard (single-focus) lens, you'll see distance clearly but will need glasses for reading. This isn't a complication — it's the expected outcome of that lens choice. What's commonly called a "smart lens" is actually a multifocal lens; it lets you see up close without glasses too. It can take your brain 1-3 months to adjust to these lenses, and halos around lights at night tend to fade during that time.

If only one of your eyes has had surgery, the prescription difference between your two eyes may bother you. A temporary fix: remove the lens on the operated side from your old glasses. If your other eye also has a cataract, it's usually operated on about 1-4 weeks later.

If your vision clouds up again months later

It's common for vision to cloud up again months or years after cataract surgery. Patients usually describe this as "my cataract came back."

Cataracts don't come back. The lens that was removed doesn't grow back. What's actually happening is that the thin, clear membrane holding the new lens in place has gradually become cloudy.

  • Vision clouds gradually and glare from lights increases — similar to the original cataract.
  • The fix is a YAG laser procedure: — it takes a few minutes, is painless, is done with eye drops, and doesn't require an operating room.
  • It's done once and doesn't recur in the same eye.
